The use and efficacy of SMART therapy in the treatment of asthma in the light of the latest GINA 2020 guidelines

Summary: Asthma is one of the most common chronic diseases of the respiratory system. By definition, asthma is a condition characterized by chronic inflammation in the airways. Its main symptoms include wheezing and shortness of breath. Although it is not possible to completely cure asthma, the correct treatment allows for good disease control. Asthma therapy includes the long-term use of disease control medications and “rescue” preparations for acute asthma symptoms. According to the current GINA guidelines, it is now recommended to use combination therapy with a low-dose glucocorticoid in combination with formoterol for both asthma control and emergency treatment (even in patients with mild asthma symptoms). This combination is used as SMART therapy (Single Maintenance and Reliever Therapy). The effectiveness of this model has been confirmed in several clinical trials (including STAY, AHEAD, COMPASS, STEAM). The paper presents case reports and the authors’ experience with the use of SMART therapy in the treatment of patients with asthma of various severity.
