Paradoxical patient reports about perceived action and side effects of antidepressants and anxiolytics: Psychological and situational context
Summary:
Every medical activity, including pharmacotherapy, has a biological dimension as well as many parallel psychological, subjective and individual aspects. The biological and psychological aspects of pharmacotherapy together determine the ultimate effectiveness of treatment given to an individual person. Two cases of patients treated in the COVID-19 pandemic are an example of the meaning of these psychological aspects of antidepressant and anxiolytic treatment in the context of paradoxical reports about side effects and treatment results. In the first case the patient, who was taking escitalopram 20 mg and pregabalin 300 mg, reported increased libido as a side effect of the medication. A careful interview with the patient revealed how happy he was to have a job, his family and home in the time of the pandemic. In the first months of the pandemic he had frightening visions about losing everything (job, family members, his home) as a result of the financial crisis. This patient had no bipolar traits or symptoms of mania. In the second case described in this paper, treatment with pregabalin was so effective that symptoms of anxiety in a patient with GAD (generalized anxiety disorder) disappeared in a very short time after the start of drug administration. The sudden change in the way the patient experienced himself and his relations led to the disappearance of fear and to drug discontinuation. This was connected with confronting his feelings previously masked by intensive anxiety.
