Summary: Pharmaceutical care is about providing effective pharmacotherapy that improves the quality of life of patients. Pharmacy services bring many benefits, such as improved treatment outcomes, increased adherence, and the potential to reduce medical costs. In Poland, pharmacists play an important role in the healthcare system through their high availability for patients in community pharmacies and the public trust they enjoy among Poles. The scope of services provided by pharmacists in Poland is narrow and their potential to improve health is currently not fully exploited. Among many interventions implemented by pharmacists, according to meta-analyses, the most beneficial for patients’ health are services related to chronic disease management, screening, immunization, smoking cessation, medicine reviews or referral (enabling continuity of treatment). Beneficial economic results were shown for majority of pharmacy interventions included in the economic evaluation. Drug review service leads to beneficial changes in drug and healthcare costs, improves patient health, control of glycated hemoglobin, blood pressure and cholesterol, but does not reduce mortality. Drug reviews are cost-effective for patients with hypertension and type 2 diabetes.
