Racecadotril for the management of acute diarrhea in children

Summary: Acute diarrhea is a common condition caused predominantly by viral infections and affecting individuals of all ages. Oral rehydration therapy (ORT) is the primary management approach, while intravenous rehydration therapy is reserved for severe dehydration cases. However, ORT alone does not significantly alter the duration and severity of diarrhea, which has led to growing interest in pharmacotherapy. This article presents an overview of acute infectious diarrhea, with a particular focus on the potential benefits of using racecadotril. Racecadotril is an antidiarrheal medication that inhibits enkephalinase, an enzyme that degrades endogenous enkephalins. By doing so, it has an antisecretory effect that decreases intestinal fluid secretion and improves stool consistency. Racecadotril is approved for use in adults, children, and infants over 3 months of age, and it should be used as an adjunct to rehydration therapy, not as a replacement.
