Nutritional therapy in Crohn's disease in children in light of current guidelines
Summary:
As epidemiologic data have associated diet with the development of inflammatory bowel diseases, there has been growing interest in the role of diet as a modifiable factor for treatment of this condition. However, very few diets have actually demonstrated clear therapeutic benefit in pediatric Crohn's disease. The most evidence-supported dietary therapy for induction of remission in luminal Crohn's disease in children is exclusive enteral nutrition, which is currently recommended by gastroenterological and nutrition societies as a first-choice treatment. The Crohn's disease exclusion diet combined with partial enteral nutrition might be considered as an attractive alternative to exclusive enteral nutrition for induction and maintenance therapy in pediatric Crohn's disease. The involvement of trained dietitians becomes crucial in the management of pediatric Crohn's disease, as they possess not only valuable knowledge about available dietary options, but also insights into patients' habits, motivation to adhere to the diet and cost of the diet.
