Systemic glucocorticoids in the treatment of severe uncontrolled asthma – the risk of side effects and alternative therapies

Summary: Glucocorticoids are endogenous hormones that are responsible for a number of metabolic processes in our body, i.e. transformations of proteins, carbohydrates and lipids. They also have a strong anti-inflammatory effect. Nowadays inhaled corticosteroids are the mainstay of asthma management and oral ones are used in the treatment of severe uncontrolled bronchial asthma and in the therapy of severe exacerbations. However, their significant clinical efficacy is associated with the high risk of numerous and serious side effects, especially in the case of long-term systemic therapy. Therefore, researchers are looking for methods that will reduce the burden of systemic glucocorticoids. Currently experts recommended biologic therapies, as steroid-sparing medicines as a first line in the treatment of severe asthma. What is important in Poland, some biological drugs are already available as part of NFZ drug programs, which considerably increases the therapeutic options available to patients.
