Summary: Recent studies have shown substantial benefit of treatment with mineralocorticosteroid receptor antagonists, in particular with spironolactone, in different clinical situations, in patients with difficult-to-treat hypertension. The review shows the place of spironolactone in the treatment of hypertensive subjects according to the newest European Society of Cardiology and European Society of Hypertension guidelines.
