Hypertension treatment and risk of cognitive impairment
Summary:
Hypertension is one of the major modifiable risk factors for the development of dementia. Antihypertensive drugs are the only drugs that have been clearly shown to reduce the risk of developing dementia. This was also confirmed in the most recent meta-analysis which included elderly patients. The management of elderly hypertension in patients in terms of reducing the risk of developing cognitive disorders should include not only lowering blood pressure but also influencing pulse pressure and blood pressure variability, which increase in this age group is associated with a higher risk of dementia. The PTNT guidelines indicate the benefits of using the combination of a calcium antagonist and a thiazide-like diuretic in this age group, especially in the form of a single pill combination. As demonstrated for the combination of amlodipine and indapamide, it is possible not only to effectively lower systolic blood pressure, but also to reduce its variability and pulse pressure. This may have the potential to reduce the risk of developing cognitive functions.
