Diagnostics and therapy of urticaria in the light of current guidelines and Polish version of Urticaria Control Test (UCT)
Summary:
Urticaria is a disease characterized by recurrent urticarial wheals, angioedema or both. Based on the length of time over which symptoms recur, urticaria is classified as acute or chronic. Acute urticaria is defined as recurrence of urticarial wheals, angioedema or both for 6 weeks or less. Chronic urticaria is defined as recurrence of these symptoms for more than 6 weeks. Based on the role of triggers, we distinguish spontaneous urticaria and inducible urticaria. The diagnostics of urticaria uses laboratory tests and, in selected cases, trigger factor evaluation tests. The goal of urticaria treatment is to achieve complete resolution of symptoms. Therapeutic management is multifaceted and should be aimed at eliminating causes and avoiding triggers, possible induction of trigger tolerance (effective in some forms of inducible urticaria) and pharmacological treatment. It is necessary to simultaneously assess disease activity, degree of control and impact on quality of life in patients with urticaria in order to obtain a complete evaluation. In assessing the degree of urticaria control, the Urticaria Control Test is useful. A Polish version of this questionnaire has been validated. The obtained score ranges are helpful in deciding whether to intensify, maintain or reduce current treatment.
