Diabetic neuropathy - diagnosis and treatment

Summary: Diabetic neuropathy is the most common chronic complication of diabetes mellitus. All individuals with type 2 diabetes mellitus should be screened for diabetic peripheral neuropathy annually. Symptoms of distal symmetric polyneuropathy include dysesthesia (burning or tingling sensations), numbness, and pain. Physical examination reveals reduced or absent perception of touch, vibration, pain, temperature, impaired reflexes, and reduced muscle strength. Clinical manifestations of autonomic neuropathy include hypoglycaemia unawareness, resting tachycardia, orthostatic hypotension, gastroparesis, constipation, diarrhoea, erectile dysfunction, increased or decreased sweating, and neurogenic bladder. The treatment includes proper glycaemic control, management of hypertension and hyperlipidaemia, alpha-lipoic acid, benfotiamine, and ACE inhibitors. Pain associated with distal symmetrical polyneuropathy may be managed with serotonin-norepinephrine reuptake inhibitors, tricyclic antidepressants, anticonvulsants, or opioids. The treatment of diabetic autonomic neuropathy is complex and dependent on prevailing symptoms.
