Persistent constipation

Summary: Constipation, one of the geriatric giants, often occurs in elderly patients, negatively affects the quality of life, and can lead to polypharmacy. To a large extent, it results from pharmacotherapy used chronically; therefore, a review of the medications taken by the patient should be the first diagnostic step. The patient's essential evaluation is based on a thorough medical history, physical examination, digital rectal exam (DRE), and peripheral blood count assessment. If alarm symptoms occur, imaging and endoscopic diagnostics are necessary in order to exclude colon cancer. The basis of therapeutic management is non-pharmacological treatment, including dietary advice and an increase in physical activity. Osmotic laxatives, bulking agents and stool softeners are generally well tolerated and can be used chronically. Stimulant laxatives should be used only if necessary. If there is no improvement after the initial treatment, the diagnosis of defecatory disorders and colonic transit and appropriate treatment implementation is recommended.
