Recommendations for the use of budesonide by nebulization in SARS-CoV-2 virus-infected patients with COVID-19 symptoms. Expert position

Summary: In the case of SARS-Cov-2 infection, the most common route of infection is respiration. Interstitial pneumonia is the main cause of death among these patients. It has been shown that the inclusion of inhaled glucocorticosteroids in patients infected with SARS-Cov-2 in the early period of infection is an effective and safe intervention reducing the risk of hospitalization, alleviating symptoms and accelerating the recovery of patients. Inhaled budesonide can be used in all people, regardless of the severity of symptoms, age and health burden. No serious side effects were observed, which distinguishes it from other drugs used in COVID. The use of the drug in the form of nebulization is a simple and reliable method of delivering the drug to the place of its target action, which makes it highly effective, especially in people who are inexperienced with the use of inhalers, in small children and the elderly, and patients with severe shortness of breath. While taking precautions (using the right type of nebulizer, using antiviral filters, keeping the right distance, airing the room), also a safe method. Budesonide nebuliser suspension is available for outpatient treatment and is inexpensive compared to other COVID-19 treatments. Therefore, there are no obstacles and limitations to its widespread use not only in patients with bronchial asthma, COPD and croup syndrome, but in every person infected with SARS-Cov-2 with respiratory symptoms at risk of disease progression towards a severe form. The earlier this therapy is applied, the better its effects will be. There will be fewer patients requiring hospitalization and thus less burden on health care resources, which was a significant problem in this epidemic and not only concerned providing care for infected patients, but also for other patients with other diseases, for whom there was no room in hospitals.
